NoteA guide only. Response to different opioids varies. Reassess closely after switching and adjust the dose. Methadone conversion is not included; restrict it to specialists. Fentanyl patch uses a factor of 3.6 (12 µg/h ≈ 43 mg oral morphine/24 h, the upper part of the guide’s 30–45 mg range); sublingual buprenorphine (8–16 mg) uses the mid-point.
SourceConversion values are taken from the WA Department of Health Opioid Conversion Guide (OCM-14654, April 2026). The 25% cross-tolerance reduction is common practice and is not part of that guide. This is an independent tool, not published, affiliated with or endorsed by the Department.
